Information request form

You can expect a response from us in line with the Official Information Act (OIA) and all other relevant legislation. The OIA requires us to give you our decision on your request as soon as possible, and no later than 20 working days after we receive your request. Use the Official Information Calculator(external link) on the Office of the Ombudsman's website to find the deadline for our response.

Definition of an authorised requester

Authorised requesters include: the insured/claimant, claim contact, current property owner with Deed of Assignment (DOA), or authorised representatives including real estate agents representing the seller, with a listing agreement or requests related to you personally

Unauthorised requesters include: a potential home buyer, current property owner without a DOA, or other non-claim contact parties with demonstrated interest in receiving information.
